By completing this Pro Business Card order form you agree to the following conditions:

You are a current PSIA-AASI member and thus have the right to use the PSIA-AASI logos for personal promotion reasons;
The certifications, accreditations, awards, titles, etc. you are listing on your card are true and accurate;
You understand that failure to meet the above criteria is a potential ethics violation which could lead to censure and/or expulsion from your Region and the National organization.
